Edward P. Abraham, MD, FRCSC, is an Associate Professor of Surgery in the Department of Orthopaedics at Dalhousie University Medical School, Saint John Campus, Saint John, NB, Spine Centre Horizon Health Network.

Philip Chan, MD, FRCPC (Anesthesiology, Pain Medicine), FIPP, is a Director at the Chronic Pain Clinic in the Department of Anesthesia/Chronic Pain Clinic at St. Joseph's Healthcare in Hamilton, Ontario. He is also an Assistant Clinical Professor in the Department of Anaesthesia, Faculty of Health Sciences at McMaster University and Program Director of Pain Medicine Residency Program at McMaster University and the Medical Director of the Neuromodulation Program at Hamilton Health Sciences Corporation in Hamilton, Ontario.

Dean Elterman, MD, FRCSC, is an Assistant Professor at the University of Toronto, Toronto Western Hospital in Toronto, Ontario.

Alfonso Fasano, MD, PhD, is on staff at the Morton and Gloria Shulman Movement Disorders Clinic and the Edmond J. Safra Program in Parkinson's Disease at Toronto Western Hospital and Division of Neurology, University of Toronto, Toronto, Ontario, Krembil Research Institute, Toronto, Ontario.

Michael Gordon, MD, MSc, FRCPC, FACP, FRCPEdin, is a Geriatric Consultant in Palliative Care at the Baycrest Geriatric Health Care System and Professor of Medicine at the University of Toronto in Toronto, Ontario.

Hamilton Hall, MD, FRCSC, is a Professor in the Department of Surgery at the University of Toronto. He is the Medical Director, CBI Health Group and Executive Director of the Canadian Spine Society in Toronto, Ontario.

Allan Ho, MBBS, MSc, FRCS(ORLHNS), FRCSC, is an Associate Professor at the University of Alberta and the Director of the Edmonton Ear Clinic in Edmonton, Alberta.
